Just so everyone knows the status of this.
It was found that someone changed the user rights on SQL Server for the user
being used. Something simple. But no one was acknowledging they had made
a change. And this occurred on a dedicated hosted server! Why someone
who is hosting the service would make a change like this is beyond me.
But never the less, it can happen and did.
Problem solved.
